How Referrals Work
When we receive a referral, our team will:
Contact the patient or parent to help schedule an orthodontic evaluation
Collect the necessary history and records to support an accurate assessment
Provide a clear treatment recommendation and timing guidance
Coordinate with the referring provider when collaboration is helpful
If your patient has a time-sensitive concern, please note it clearly in the referral details so we can prioritize appropriately.
Information to Include With a Referral
To help us evaluate efficiently, please provide as much of the following as possible:
Patient name, date of birth, and preferred contact information
Parent or guardian details for minors
Reason for referral and primary orthodontic concern
Relevant dental history, including trauma, missing teeth, or eruption issues
Current dental treatment in progress, if applicable
Any periodontal or restorative considerations that may affect timing
Records and Imaging
If available, you may include:
Recent panoramic radiograph and cephalometric radiograph
Intraoral photos and facial photos
Current study models or digital scans
Notes related to space management, eruption patterns, or impacted teeth
If records are not available, we can take the necessary images and photos during the evaluation.
Communication and Coordination
We believe referrals work best with clear communication. Following the patient’s visit, we can provide an update on findings, recommended timing, and treatment direction, especially when interdisciplinary planning is beneficial.
